    Kshara Karma

Kshara Karma is an Ayurvedic para-surgical procedure using herbal alkaline preparations (kshara) for cauterization and treatment of conditions like hemorrhoids, fistula, and warts.

    Overview

Kshara Karma involves the application of caustic herbal pastes or powders (kshara) to diseased tissue for therapeutic destruction and healing. It is one of the ancient para-surgical techniques of Ayurveda, described alongside Agni Karma (thermal cauterization) and Shastra Karma (instrumental surgery). The therapy works by chemically cauterizing the affected tissue, promoting controlled tissue destruction followed by regeneration.

    History and Legend

Kshara Karma is described extensively in the Sushruta Samhitaon/sushruta) Samhita, which details the preparation of different kshara from various plant ashes (yava kshara, tankana kshara, audbida kshara). Sushruta classified kshara into vana kshara (forest/plant-based), audbida kshara (mineral-based), and mixed kshara. The procedure was refined over centuries by Ayurvedic surgeons for treating Arshas (hemorrhoids), Bhagandara (fistula-in-ano), Kustha (skin diseases), and Arbuda (tumors). The tradition of Kshara Karma was preserved by traditional surgical lineages in Gujarat and Kerala.

    Architecture and Features

Kshara is prepared by burning specific plants (like barley, sesame stalks, or specific herbs) and collecting the ash, which is then processed into a potent alkaline powder or paste. The kshara is applied to the diseased tissue using specialized applicators, left for a specified duration, and then neutralized. The procedure requires knowledge of tissue tolerance and precise timing to prevent excessive damage. Modern Ayurvedic surgery departments use standardized kshara preparations with quality control.

    Modern Status

Kshara Karma is practiced at specialized Ayurveda surgical departments in institutions like the Gujarat Ayurveda University, the Institute of Post Graduate Ayurveda Education in Varanasi, and government Ayurveda hospitals. Research has demonstrated its effectiveness for hemorrhoids and fistula with lower recurrence rates compared to conventional surgery. The procedure is included in the BAMS (Bachelor of Ayurvedic Medicine and Surgery) surgical training curriculum.
